Abstract: 

Backgroundm: Based on recent evidence, inadequate and ineffective follow up after discharge increases the risk of unplanned READMISSIONs, which is an indicator of poor healthcare system function. The aim of this study was to determine the effects of PATIENT education and follow up after discharge on hospital READMISSION in heart failure PATIENTs.Methods: This randomized clinical trial was conducted on 120 PATIENTs with heart failure admitted in Taleghani hospital, Tehran and were allocated equally with block randomization method. The intervention group were educated based on recent guidelines and their needs after discharge and were followed up for 3 months by 9 telephone interviews (every week for the first month after discharge, every two weeks for the next two months), but the control group received the routine care. Data were gathered based on a control check-list every two weeks for both groups and were analyzed using chi-square test, Fisher and t-test. Evaluation of intervention effect was done using generalized estimating equations (GEE) model.Results: READMISSION rate was 19.3% in intervention group and 38.2% in control group (p=0.05). The PATIENTs that needed outPATIENT care did not differ significantly in both groups (17.5% in intervention group vs.28.3% in control group).Conclusion: This study suggests that PATIENT education and follow up after discharge might decrease READMISSION rate among heart failure PATIENTs.

Abstract: 

Context: In response to the growing strain on healthcare resources, urgent measures are needed to encourage early discharge and prevent unnecessary hospital READMISSIONs. This review aimed to systematically assess studies of interventions designed to reduce READMISSIONs. Methods: The search was conducted between February 20 and March 20, 2023. Original articles focusing on interventions to reduce hospital READMISSION rates were reviewed. Two researchers were separately responsible for selecting articles, extracting data, and conducting qualitative assessments using the STROBE and CONSORT guidelines. Results: The study included a total of seventy-two articles. The proposed interventions for minimizing READMISSION rates were classified into 14 distinct groups. Conclusions: PATIENT follow-up after discharge and education emerge as two effective approaches in this context. Moreover, robust evidence exists to support the efficacy of community-based interventions in decreasing hospital READMISSIONs.

Abstract: 

Introduction: READMISSION to hospital because of the impact on the cost and quality of hospital care and Impose an additional burden on the healthcare system, Is an important priority for hospital managers. The aim of Study Was causes of READMISSION PATIENTs toxicological ward of the loghman hakim hospital, Research in Tehran.Method: This is an applied cross-sectional retrospective study. Research community included Admissions in 1393 in toxicological ward of the loghman hakim hospital. Research Size based on Morgan is 300 clinical records. Sampling Method was simple randomly. READMISSION in the present study in hospitalized form was more than one defined. Demographic data includes (age, gender, marital status, occupation, education and variables related to hospitalization (hospitalization Frequency, length of stay, and poisoning quality, discharge situation, referral and insurance) by means of information form was extracted from records. In two level Descriptive statistics (frequency and percentage) and inferential statistics (correlation chi-square test, dependent T test and Chi-square test) were analyzed using SPSS21 software and hypothesis testing was done.Finding: During the study period300 READMISSION cases were hospitalized in toxicological ward of the loghman hakim hospital.13/4% PATIENT was READMISSION. The READMISSION cause in 41/6% PATIENT was related to family issues. It Can be named respectively emotional, spiritual, and addiction with regard to other topics. The total cost of treating PATIENTs was 206521754 Rials. The average cost of stay per PATIENT 10256639 Rials, payment by PATIENT 928136 Rials, and Subsidies health payment by health ministry was 1834370 Rials.Conclusion: The results of this study showed that, several factors may be involved in READMISSION to hospital PATIENTs poisons ،the most important of them can be mentioned in the four ares of family problems, emotional problems, mental problems and addiction.

Abstract: 

Background and purpose: Neonatal READMISSIONs incur significant costs to the healthcare system. Identifying the causes of neonatal READMISSIONs is therefore crucial. This study aimed to identify the causes of neonatal READMISSIONs within the first two months of life at Ayatollah Mousavi Hospital in Zanjan, Iran, between 2020 and 2023. Materials and methods: This was a descriptive cross-sectional study. Out of 12,873 discharge records from Ayatollah Mousavi Hospital in Zanjan, from April 2020 to March 2023, a total of 105 neonatal READMISSION records were identified. Data were collected using demographic information, neonatal characteristics, and reasons for READMISSION. Data analysis was performed using descriptive statistical indices, McNemar’s test, the Chi-square test, and one-way analysis of variance (ANOVA). Results: The READMISSION rate was 8.15 cases per 1,000 live births. The most common causes of READMISSION were jaundice (41%) and infection (39%). The most common causes of initial hospitalization in neonates were jaundice (54.3%), respiratory distress or prematurity (21.9%), and infection (11.4%). The mean time interval from discharge to hospital READMISSION was 4.24 ± 3.90 days. None of the maternal or neonatal factors were significantly associated with hospital READMISSION (P > 0.05). Infants with jaundice were readmitted within a shorter period after discharge than those with infection. However, the length of stay during READMISSION was not significantly different between the two groups, although it was longer in the infection group. Conclusion: The most common causes of neonatal READMISSION were jaundice and infection. Implementing protocols for the assessment of jaundice and infection prior to hospital discharge is recommended to reduce the rate of neonatal READMISSIONs

Abstract: 

Introduction: READMISSION indicates partial treatment of PATIENTs except for follow-up treatments. This study aimed to determine the causes of PATIENTs’ READMISSIONs in teaching hospitals at Zahedan University of Medical Sciences in 2010.Methods: In this cross-sectional study, 864 out of 2798 medical records of PATIENTs readmitted to the hospitals were randomly selected. Data were collected using a checklist included 16 items in five sections. Validity and reliability of the checklist were assessed. Data were analyzed using the descriptive statistics, Fisher exact test, and Chi-squared test.Results: From all readmitted PATIENTs, 52.3% were females. Follow-up treatment was mentioned as the main cause of PATIENTs’ READMISSIONs (46.5%). Recurrence of disease (20.3%), disease complications (14.5%), other diseases (10.4%), partial treatment (7.1%), postoperative complications (1%), and medical procedure complications (0.2%) were other causes of READMISSIONs.Conclusion: This study showed that 56.9% of the causes of PATIENTs' READMISSIONs were inevitable; and 43.1% of them were evitable. Some proper measurements to decrease PATIENTs' READMISSIONs are equipping hospitals with adequate facilities for outPATIENT surgeries, performing non-invasive surgeries, improving outPATIENT clinics and quality of clinical services, visiting PATIENTs at home, and educating PATIENTs at the time of discharge.

Abstract: 

Objective: We aimed to describe psychiatrists' attributions on non-compliance related issues resulting in re-hospitalizations of psychiatric PATIENTs.Method: In a cross sectional study, we included 500 randomly selected psychiatric readmitted PATIENTs and registered their demographic data (including age, sex, job, marital status, and educational level), and psychiatric clinical data (including diagnosis, medications, and presence of psychiatric disorders in family members). Possible noncompliance issues by means of type and causes were asked through a structured interview by a psychiatrist.Results: Non-compliance was reported as a possible cause of admission in 441 (88.2) of the re-hospitalized cases. No insight to disease (n=295; 59%), and feeling of cure (n=138; 27.6%) were the 2 most prevalent causes for noncompliance of the PATIENTs.Conclusion: It seems that non-compliance, as a prevalent factor, possibly causes READMISSION in psychiatric wards. Providing a better insight to disease and to instruct PATIENTs to take their medications even if they have some feeling of cure is important to decrease such problems.

Abstract: 

Background & Aims: READMISSION to hospital because of the impact on the cost and quality of hospital care and Impose an additional burden on the healthcare system, Is an important priority for hospital managers. The aim of Study Was Assessment of prevalence of READMISSION in admitted ward of Tehran Oil Company’s hospital in 2016.Material & Methods: This descriptive-analytic study was performed on 260 PATIENTs hospitalized in Tehran Oilfield Hospital. The information collection tool included information such as age, gender, education, hospitalization, reasons for initial hospitalization, number of admissions, reasons for admission and hospitalization costs. Data analysis was done using SPSS version 21 and using Chi-square tests.Results: The results showed that 20% of the total hospitalization in 2016 was re-admitted. The rate of hospitalization according to the frequency of admission showed that 74.6% of the PATIENTs referred to the hospital for the second time, 12.3% for the third time and 13.1% for more than three times to follow the treatment. There was a significant relationship between the age, duration of hospitalization and the education of PATIENTs with frequency of re-admission (P<0.05).Conclusion: Considering the results of post-discharge education, the awareness of the importance of follow-up of treatment, the greater attention of the clinical staff to the health care needs of PATIENTs, the awareness of hospital administrators of reimbursement costs and comprehensive planning and prevention in Hospitals can be useful in preventing abortion because of administrative factors.

Abstract: 

Background and purpose: Despite so many advances in the treatment of heart failure, the rate of READMISSION is still relatively high which imposes additional costs on PATIENTs and hospitals. Therefore, this study was designed and implemented to determine the causes of rehospitalization of heart failure PATIENTs admitted to Fatemeh Zahra Heart Center in Sari during 2022. Materials and methods: In this descriptive study, 309 PATIENTs with heart failure, who were admitted at Fatemeh Zahra Heart center for the second time, were selected by available sampling method during 2022. The data collection tool was a questionnaire which included demographic information as well as factors associated with READMISSION of heart failure PATIENTs. The qualitative content validity of the questionnaire was checked and confirmed by five cardiologists. The samples were analyzed using SPSS V21 applying descriptive statistics of relative frequency, absolute frequency, mean, and standard deviation. Results: Dyspnea was reported in 88% of the PATIENTs, and according to the NYHA Class evaluation criteria, the highest level of severity was 2 (in 44% of cases). An increase in chest pain before rehospitalization was also reported in 30% of the PATIENTs, and according to the CCS Class evaluation criteria, the highest level of severity was 2 (in 54% of cases) Failure to comply with the rehabilitation program was observed among 74% of PATIENTs who had a rehabilitation program. The other main reasons for READMISSION of heart failure PATIENTs in order of priority were failure to comply with medication regimen (28% of PATIENTs), palpitation (28%), stressful event (22%), heavy activity (21%), non-adherence to food diet (21%), smoking (7%), infectious disease (4%), discontinuation of medication (4%), and alcohol use (3%). Conclusion.: Considering that dyspnea was the main reason for rehospitalization of heart failure PATIENTs, and non-compliance with the rehabilitation program, non-observance of the drug, and food diet regimen accounted for a high percentage of READMISSION, it is recommended to emphasize compliance with the rehabilitation program due to its great impact on improving the respiratory condition. Furthermore, it is beneficial to educate PATIENTs on adhereing to their diet and medication, using consultations, and following the doctors' recommendations after discharge. This can be effective in preventing rehospitalization of PATIENTs and improving the quality of their life.
